Previously: Panhandlers Bertie and Rebecca went to live with their niece, but didn't seem very happy about it. Kem gave Carter her father's necklace or bracelet or something.
Carter stands and watches construction on the new clinic. Abby walks up and asks if he's surveying his kingdom, and Carter says that he was actually wondering what they do with all the dirt. Haven't you ever seen those signs that say "Clean Fill Wanted"? That's what they do with all the dirt. Abby asks when the clinic will open, and Carter says it will be a year from September. It's nice to know that even Carter's gazillions can't buy speedy construction. Abby congratulates Carter on getting tenure, but Carter doesn't seem very excited about it. Hee! In the background of the shot is a PortaPotty called "Oui Oui." Abby asks if tenure isn't a good thing, and Carter says without conviction that it's great. He asks Abby if she's ready to complete her internship, and become an R2. Abby doesn't seem ready, and says that her internship year went quickly. She asks who Joshua Carter is, and Carter claims it's just a family member. Abby digs a little deeper and gets Carter to reveal that it's the name he and Kem were going to give their baby. Abby is silent, because what do you say to that? Finally, she tells Carter that he's doing a good thing in building the clinic.
Inside the hospital, Susan tells Morris that, now that he's Chief Resident, the other departments will call and yell at him when something goes wrong. So Morris will get yelled at by other doctors? How is that any different? Susan mentions office hours, and Morris gets all excited about having an office. Susan tries to get him back on track, since her point is that he needs to be available for questions and concerns from the residents. Susan finishes up by telling Morris that he needs to be "head cheerleader" and organize happy hours and the like. Morris seems perturbed about this part of his job. Susan hands out the resident contracts for year to Neela and Abby, but tells Shane that they need to talk about his. Shane is confused by that one. So am I.
Luka and Sam are in a therapist's office. The therapist determines that Sam and Luka work together, and that they've been together for about a year. That's all? It seems like they've been together longer. Probably due to the drudgery of their relationship. Sam is doing all the talking, although she pauses a few times to let Luka jump in, but Luka doesn't speak. The therapist asks if they've sought therapy before, and Luka and Sam both shake their heads no. The therapist asks why they came. Sam pauses and looks over at Luka to see if he'll answer. Luka just sits there, so Sam hesitantly says, "I...I don't think Luka and I should be together." So just break up. It's not like they're married, or have a child together. Why does Luka want to be with Sam if she doesn't want to be with him?
We return from the credits with a close-up of a dude who's been scalped. Understandably, he's freaking out. And very bloody. Pratt and Neela try to hold the guy down and examine him. Carter walks in and asks what's up. Neela explains that Scalpy McNoHair was checking his radiator while his engine was on, and his hair got caught in the belt. Carter picks up the piece of scalp that was removed. Yikes. Neela adds that the guy tried to pull his hair out of the belt with his hand, so there's extensive damage to that hand, too. Pratt quizzes Neela about the medication she wants, and she orders an antibiotic. Pratt asks if there's anything else, and Carter and Pratt stare at Neela, who looks terrified. Finally, Pratt just places the order himself. Frank walks in and says that a friend of Pratt's is in the waiting room. Carter tells Pratt to go ahead.
Pratt walks into the waiting room and greets his friend, Darnell, with a hug. Darnell points out his brother or son or friend, KJ, a surly teenager. Pratt says hello, and Darnell asks if he can speak to Pratt privately.
Back at the therapist's office, Luka says that he's happy, although he sure doesn't sound very happy. Sam tells Luka he's not happy, because no one is happy all the time. Luka sighs, frustrated. Sam tells the therapist that Luka is "like a sphinx," and that she never knows what he's thinking. Luka protests that they talk, and Sam says that she talks, and Luka smiles and nods. Sam says she's always the one getting angry and yelling. Luka asks if he's supposed to get angry. Sam says it would be nice if he broke a dish or screamed once in a while. So Sam assumes that because Luka doesn't act like a child, he's not angry. And Luka assumes that Sam can read his mind. Great relationship. Luka sighs again, and Sam says that this is exactly what she's talking about. The therapist asks Luka what he's feeling. Luka says he's happy and Sam's not and he doesn't know why. He thinks they're healthy, and that they enjoy each other and their work, and have a good life. Sam looks sad and shakes her head. The therapist asks Sam how that makes her feel, and Sam says it makes her feel "like hitting him over the head with a hammer." They hate each other! Just break up! The therapist asks if there was a particular event that led them to come for therapy. Sam says that she thought she was pregnant, but is greatly relieved that she's not. Luka looks at her sharply. The therapist comments on Sam's great relief, and Sam says that's how she feels. The therapist asks Luka how he feels, and Luka doesn't answer.
In the hospital, something really gross and brown drips from the ceiling onto a patient's chest. The patient freaks out and says it smells like crap. He smells it and says that it is crap. Abby and Neela move his bed, and Neela says that it must be his DTs kicking in. The patient screams at some paramedics wheeling in a patient that they should leave and go to Mercy instead. Abby pulls a curtain shut around the yelling guy. Neela asks if Jake is in Philadelphia, and Abby says he's at Tulane. Neela asks about San Francisco, and Abby says that was a few weeks ago. Abby reports the dripping crap to Frank. Neela asks what's up with Carter getting tenure and Susan getting denied. Morris walks though and says, "Bada bing. Show me the green." Frank says that Carter's "African love" is on the phone, and Neela says he's still working on the guy who got scalped, so Frank takes a message. Neela asks the room in general if you get tenure based on how much money you bring in. Neela is just learning this? Shane says that Carter is building a new wing for the hospital. Chuny calls out that Susan is a highly-qualified woman and Carter is a well-liked man. Frank says, "Oh, put your bra back on, Gloria Steinem. Carter's been here ten years. Lewis split and then came back." I don't usually like Frank's sexist and racist garbage, but that was pretty funny. And also, have the writers been reading the forums, because I swear the posters had this same conversation last week. Morris tells everyone to cheese it because Susan is heading their way. Susan walks through and asks Shane to talk to her about his contract. Shane follows her out.
Pratt is talking to Darnell about KJ working in the hospital. Apparently, KJ needs to do community-service hours because he got busted for "tagging" and "he had some weed on him too." Pratt sighs, and Darnell says that they did a lot worse when they were kids, but never got caught. Pratt says that he's "just a hired hand," so it's not his decision. Darnell asks if Pratt will ask, and Pratt agrees.
Back in the therapist's office, Sam is saying she doesn't think having another kid would be fair to Alex. Luka thinks Alex would like to have a family. Since we last saw them, Sam and Luka have moved about two more feet apart, and are now sitting as far from each other as they can while still being on the same couch. The therapist tries to bring up how complex it can be adding a child to a blended family. Sam bitches that she didn't even want to have Alex. I'm sure that attitude has nothing to do with Alex's problems or anything. Sam tells the therapist that she was fifteen. Luka says she's not anymore. Sam goes for the low blow and asks, "After what happened to your wife and kids, you really want to bring another baby into this world?" Luka shoots her a look like, "Back off the dead family, sister." The therapist asks what they're talking about, and Luka won't speak. Sam points out that if Luka were happy, he could talk about it. Luka says quickly that he had a wife and kids, but that they were killed years ago, and he doesn't see the point in talking about it. Sam says it's a huge part of who Luka is, and he won't let her in. Maybe it's because I'm a talker, but I'm starting to take Sam's side in this.
In the hospital, Abby asks why every kid named Destiny, Miracle, or Faith ends up with leukemia. Carter checks a chart and gives instructions. Frank tells Carter that he got a call from his "Kenyan love nest." Carter says that it's the Congo, and that he knows Frank knows that. Frank jokes that it's all "mosquitoes the size of toasters." He adds that Kem sounded kind of upset. A janitor puts a bucket on the floor to catch the raw sewage dripping from the ceiling, and says it'll only be another hour or so. Abby jokes that when the bucket's full, they can just toss it out in the street "like [they] do the chamber pots." It was mostly the delivery, but that was a funny line. Then again, I'm five so I think poop is always funny. Carter calls Kem and gets Charles. Morris walks by and tells Frank to move some boxes out of his new office. Carter is apparently getting bad news from Charles, as he asks worriedly, "What happened?"
Susan asks Shane why he's still working there, taking up a slot someone else could have. Shane asks if Susan thinks he's a bad doctor. Susan says Shane could be good, but that he's a clock-watcher. She suggests that he take his degree and go work somewhere else, since he's just doing it to make a living and support his music. Shane doesn't answer. Susan tells him not to sign the contract unless he's sure this is what he wants to do. Shane sighs.
Carter puts on his overcoat. Abby pops into the lounge and asks if everything's okay. Carter says that Kem's mother, who lives in Paris, is in the hospital. That's about all Carter knows. He busts out of the lounge and tells Frank to inform Susan that he had to go, and thathe'll call from the airport. No wonder this hospital has low patient scores. Apparently, people can just leave whenever.
Sam and Luka walk out of the therapist's office. Luka is talking about running errands. He says that he's going to stop by Home Depot and find a new towel bar to replace the one Alex pulled down. Sam realizes that, in Luka's mind, nothing has changed and the therapy had no effect, so she storms off and refuses to talk to him. Immature, yes, but I get where she's coming from. Luka is totally bewildered by Sam's behavior.
Carter is in Paris, riding in the back of a taxi. That's the whole scene.
It's Tuesday morning in the hospital. Susan explains to Luka that Kem's mother has meningitis, which she contracted because her immune system has been weakened by chemo. Neela and Abby complain to Morris about their new work schedules. As Morris and Shane bond over their upcoming trip to see the Cubs play, Neela points out that Shane has every weekend off. Abby asks if Shane bribed Morris for a good schedule. Morris denies it, and then says that he'd love to go see U2 but he couldn't get tickets. Neela turns to complain to Susan, but she just sighs and says that Morris is the Chief. She walks over to Luka, who asks if they could make Abby Chief Resident. Susan says that Morris is "a paperwork hound," plus his new job keeps him from seeing patients. Sam walks up and writes something on the board, and then walks away without even glancing at Luka. Weren't they supposed to work different shifts if they can't get along? Jerry notices the cold shoulder and recommends that Luka buy flowers. Pratt shows up with KJ, and introduces him to Jerry as their new volunteer. He suggests that KJ run some specimens up to the lab. KJ is grossed out by the idea of blood, urine, and stool specimens. Pratt tells him he can leave if he doesn't like it.
Pratt waits outside for a septic newborn to arrive by ambulance. Neela joins him, and Pratt berates her for not knowing her antibiotics by now, as she proved during the scalping trauma yesterday. Where is this coming from? Neela has always been book-smart, so it's a little weird that suddenly she doesn't know stuff she could learn from a book. The paramedics deliver a newborn into Neela's arms. Well, not deliver, but you know what I mean. The mother hops out and says that the fever started this morning, and that she thought it was just the flu. The paramedic tells Neela that they had a line in the baby's hand, but that it blew out.
In Paris, Carter takes his bags out of the taxi's trunk and slowly walks into a hospital. Inside, he wanders down a hallway looking exhausted. He finds a desk with no one sitting behind it. He sees a nurse and asks if she speaks English. She doesn't. Carter uses his weak French skills to find out that Kem's mom is down the hall.
Carter finds the correct room, but Kem isn't there. Her bag is, though. He checks Ma Kem's IV bags to see what meds she's on. Ma Kem is asleep in the bed. Carter walks back out into the hall, and Kem calls out to him. They kiss hello; she asks what he's doing there, and says he should have told her he was coming. Oh. That's a bad sign. Carter asks how Ma Kem is doing, and says he couldn't tell which antibiotics she's on. Kem says she's not on any, because it's viral meningitis. Carter rolls his eyes and realizes that Ma Kem is okay, and that it's not that serious. A dude named Michel walks up, and Kem introduces him as "an old friend." Carter feels like a douche.
Pratt and Neela work on the newborn. Neela intubates while Pratt puts in an IV. Neela rattles off the medicine she plans to administer, and Pratt approves. Malik (Malik! Where have you been lately?) runs in and tells Pratt that another patient is crashing. Pratt promises to return quickly. Neela tells the baby's mother that they suspect an infection, so they're giving her medicine and hooking her up to a respirator. Chuny reports that the IV line is infiltrated and won't flush. Neela asks how old the baby is, and the mother says she's eight days old. Neela notes that the umbilical stump is still attached, and decides to put in an umbilical line. Have I mentioned how much belly buttons freak me out? Chuny looks worried about Neela's plan, but doesn't say anything.
Jerry tells Pratt that KJ is kind of just sitting around and doesn't seem very interested. Pratt says that KJ isn't interested. Jerry asks why he would volunteer, then, and Pratt syas that it's community service. Jerry asks what the crime was, and Pratt lies that it was second-degree murder. Morris freaks out, and Pratt admits that KJ tried to tag a police car, while the cop was inside. Wow. Maybe he deserves to go to jail. Stupid people jail. Abby brings in an x-ray to show Pratt that a six-year-old ate a huge tub of popcorn by himself. Pratt isn't interested. Abby stomps off, and Morris asks if they'll be ready to supervise interns in a few weeks. Pratt thinks there's no chance.
Carter walks out of the hospital. Kem chases him down and asks where he's going. Carter says that the whole scene was awkward, so he's headed back to the airport. Kem says that she went to school with Michel, and that he's fond her her mother. Carter asks if they're a couple. Kem says that they were. Carter asks if Michel knows it's in the past. Kem asks if Carter is jealous, and he admits that he is. Kem laughs at him, and Carter tells her not to. Carter asks if she and Michel are sleeping together, but then tells Kem not to answer that. Kem asks if Carter has been dating. Good point, Kem. Carter doesn't answer the question (Answer the question, Claire!) and instead starts trying to hail a taxi. Carter says that he's tired, he's sorry for barging in on them, and he feels ridiculous. Kem thinks it was sweet of him to come. Carter asks Kem to call him when she gets home. Carter finally gets what he wants when Kem asks him to stay, at least for one night.
Back at County, Neela is working on the umbilical line. Pratt busts in and asks what she's doing, all angry. Neela thought it was their best option at access, but Pratt says they could have just reinserted his IV. Neela starts to doubt herself, and the mother asks if Neela screwed up. Pratt doesn't answer. Way to set yourself up for a lawsuit, dude. And why is Neela suddenly the new Morris?
Kem drops Carter off at his hotel and promises to meet him for breakfast the day. Carter bitterly asks if Michel (who is driving the car but apparently doesn't speak any English) will be at breakfast too. Kem ignores his childish behavior and tells him to have a good night. Carter watches Kem drive off with Michel.
Pratt has finally caught his snap and pulled Neela outside to tell her how much she sucks, instead of doing it in front of the baby's mom. He details all of the things that can go wrong when doing an umbilical line. Neela tries to say that she did a bunch of them on her neonatology rotation, and Pratt points out that that was two years ago. Neela reminds Pratt that he told her to take more initiative, and Pratt tells her to try using common sense. He yells at her to know when to ask for help. Neela's face falls, and Pratt is disgusted that she's crying. He yells at her to start using her head. Neela walks off, still crying.
Carter wakes up before daybreak in his Paris hotel room, because his phone is ringing. It's Kem, telling him to get up.
Moments later, Carter stands outside on the street. Kem pulls up in her little car and says she wants to show Carter something. He asks if it involves coffee, and Kem says that everything in Paris involves coffee. I'm so moving to Paris. Carter gets into the car and Kem peels out.
Kem and Carter run to get on a train. Carter wonders what's going on, and Kem tells him to be patient. They play the game where you look at strangers and make up stories about their lives. I love that game. Although Kem's stories are kind of depressing, and always seem to involve someone dead or dying. Carter asks if he and Kem have a story. Kem doesn't answer, and tells Carter to close his eyes and wait. The train comes out of a tunnel, and Carter sees the sun coming up and the Eiffel Tower in the distance.
Back in Chicago, it's Wednesday morning. Neela stands outside the hospital, staring at the entrance. Abby walks up and asks what Neela is doing. Neela is deciding if she's sick, because Pratt is working. Jake walks up and hands Abby a pastry, and then asks what they're doing. Abby says that Neela is trying to decide whether she should go to work, and explains what happened with Pratt. Jake is impressed, but needs to go inside and get to work. Abby follows. Neela hops out of the way to avoid getting hit by an ambulance, then starts to follow them inside.
Morris is trying to convince Susan to paint his office, but she's so not interested. Susan tells Pratt to handle a stabbing victim, and passes off a nine-year-old kid who was in a car accident to Luka. Sam reluctantly follows Luka to the trauma.
Shane and Neela work on the stabbing victim. Shane removes her neck dressing to reveal a deep wound. Neela thinks they should intubate first. Neela moves to intubate while Shane examines the wound and realizes that the jugular was hit. Bertie and Rebecca are watching from the room. Pratt runs in, and Shane explains what's going on. Pratt looks proud as Shane and Neela continue working, and says that there might be hope for them after all.
Carter and Kem sit at an outdoor café. Kem congratulates Carter on tenure, and Carter tells her that Susan didn't get it, and is pretty pissed. Carter watches Kem and laughs because she has a lot of jam on her face. He wipes it off. A little anvil...I mean, "girl" walks up and says hello. Carter and Kem talk to her, but she's silent. Her mother walks up and pulls the little girl away, apologizing. Kem and Carter don't talk about what's on their minds, and it starts with bay, and ends with aby.
Jake finds Abby and says he needs some advice about his future job prospects, but Abby is really busy. A patient points out that there's more poop leaking from the ceiling. Jake and Abby move the patient and move on. Abby tells Susan that they have "code brown, dripping from the ceiling." Susan is pissed, and yells that they are closing to trauma until someone deals with "the raw sewage leaking from the ceiling." Should she be yelling that? Again, lawsuit.
Abby and Jake take on an elderly man with emphysema who is having trouble breathing. The man reports that he stopped smoking five years ago. Jake helps the man into a bed while he tells Abby that he was thinking of ranking County as his first choice. Abby is surprised that Jake wants to stay in Chicago. Jake says that his friends and family are all there. Abby orders Albuterol for her patient, and then asks Jake about San Francisco. Jake thinks that County has a lot to offer, and they smile at each other. Chuny tells Abby that Bertie and Rebecca are back. Abby places her orders and asks Jake if they can talk about his decision later. Jake agrees.
Abby finds Bertie and Rebecca and asks why they're back. Turns out that Rebecca was strangled. Abby lectures that it's too dangerous for them to be on the streets. Rebecca says that Bertie saved her life by stabbing her assailant. Abby is shocked. Bertie says she had to, and asks if she'll be all right. Abby thinks she's talking about Rebecca, and says that she seems stable. Bertie clarifies that she was talking about their niece, Shelley. It seems that Shelley is the stabbing victim. I thought it was going to turn out that Bertie and Rebecca were crazy, but that's not it.
Kem and Carter walk along the water as Kem talks about her childhood in Paris. Carter thinks it sounds idyllic, and wonders why Kem's family left. Kem says that her white mother and black father had a hard time. Carter mentions that their baby would be a year old now. Kem is silent, and then says that she's going to head back to the hospital. Carter just nods, and they walk along, until he grabs her arm kind of roughly and stops her. He asks urgently, "What are we doing? What are we doing? I still love you and I know you still love me. We talk to each other on the phone for hours every day. We tell each other absolutely everything about our lives. I make you happy. I make you laugh." Kem says that she tried living in Chicago, and that it didn't work out. Carter says that they don't have to live in Chicago. Kem points out that his life and career are there. Carter says that he would give it up for her. Kem says that she doesn't want him to. Carter, in his best doctor voice, says, "Kem, we had a baby, and he died. It's a tragedy that happens to people and it happened to us. But it doesn't have to define us. It doesn't. We're stronger than that. Aren't we?" Kem starts crying and says she can't. Carter says that they can have another baby. Kem starts sobbing and tells him to stop, and then says that she's sorry. Kem gathers herself together and asks Carter if he's going to come to the hospital so her mother can say goodbye. Carter looks pained and confused, but he walks off with Kem.
Shane and Neela are still working on Shelley and her stab wounds. Abby bursts in and asks Shane if he ever followed up with Social Services on Bertie and Rebecca's case. Shane is kind of confused as to why Abby is bringing this up now, in the middle of a trauma, and Abby explains that Bertie stabbed Shelley because Shelley was trying to strangle Rebecca. Shane says that he called Social Services, but that he can't follow them around all day. Shelley loses her pulse, so they start compressions and the conversation gets interrupted. Shane asks if Shelley has a cardiac history and Abby snipes, "You're the only one that's ever met her."
Carter stands at the foot of Ma Kem's hospital bed as she laughs with Kem and Michel. Ma Kem notices Carter there and asks Kem and Michel to get her some lunch. Kem and Michel leave. Ma Kem thanks Carter for her flowers, and he walks up to her bedside. Ma Kem says that Michel is handsome, but something of a fool. Carter doesn't comment. Ma Kem asks when Carter is going home, and Carter says he's leaving that night. Ma Kem thinks he's giving up too easily, and Carter says he was "rather firmly discouraged from continuing to try." Ma Kem sighs and says that life is "joy and grief hand in hand." She notices her husband's bracelet on Carter's wrist and says that you can't have true happiness until you have true sorrow for contrast, and Kem hasn't had that yet. Kem walks back in and tells her mother to leave Carter alone.
Pratt uses a rib spreader to open Shelley's chest, with the expectation that she's bleeding internally due to the angle of the stab wound. But there's no blood. Pratt tells Neela to hold compressions as he checks Shelley's heart. He reports that the coronary artery is full of air, and disgustedly calls the time of death. Neela asks how Shelley got so much air in her artery, and Pratt says that no air should have entered, due to the dressing. Shane admits that he took the dressing off to explore the neck before she was intubated. Jake interrupts the drama to tell Abby that her emphysema guy is coding, and Abby takes off. Shane tries to explain his error, but Pratt can't listen to it, and walks off.
Susan is on the phone, yelling at someone about the "literal crap pouring out of [her] ceiling." Abby and Jake start to work on the emphysema patient. Abby intubates. Pratt walks up and Abby tells him what happened. Pratt joins the crowd working on the patient.
Neela walks in and checks on Rebecca. Bertie cries that she didn't mean to hurt Shelley, and just wanted her to leave Rebecca alone. Rebecca asks if Shelley suffered. Neela says that she didn't. Bertie says that she didn't mean to kill her, and that she and Rebecca loved her.
Susan walks up to the emphysema guy and asks what's wrong. Pratt says that Abby had the patient on continuous Albuterol. Abby explains that he was "wheezing and tight." Abby says that the guy was apneic, but she doesn't know why. Susan tries to get Abby to figure it out, and Abby realizes that her treatment caused the guy to code. Susan says that the guy will be fine in a while, and that it's "a classic intern mistake" that Abby won't make again. Abby looks disappointed in herself, and Susan looks disappointed too.
Neela walks into Shelley's trauma room and yells at Shane to get up. He follows her into the hall. Neela insists that Shane tell Bertie and Rebecca what happened, because Bertie thinks she murdered her niece. Shane sighs and walks into the room as Neela watches.
Carter rides in a taxi to the airport. It's pouring rain outside. Carter is crying.
Abby sits outside the ER on a bench, looking fried. Jake walks up and says that if it's any consolation, that was a great teaching case for him. Abby says she's glad her public humiliation furthered his education. Jake says that the guy's fine, and that it's not Abby's fault he smoked for decades. Jake clears his throat and says again that he was thinking he'd rank County as his first choice. Abby asks about San Francisco, and Jake says he wants to stay at County. He smiles at Abby, who shakes her head and tells him not to do that for her. Jake looks peeved.
Shane and Neela walk out and interrupt. Shane says that it was "a seriously crappy day," and suggests that they all go get drunk and pick a fight with a bunch of law clerks. I would pay good money to see Neela in a bar fight. Also, way to read the room, Shane. Abby says that she has more charting to do, and asks what happened with Bertie and Rebecca. Neela reports that Shelley forced them to panhandle to support her crack habit, and beat them up when they didn't bring home enough money. Morris walks out and makes fun of Neela, Shane, and Abby for all their errors of the past two days. Pratt follows and says that the new interns start in a week. Morris chuckles, "God help your patients." Shut up, Morris. And why is Pratt hanging out with Morris? Neela says that she feels really inspired, and vows to go throw herself under a train. Jake starts to walk away too, and Abby asks him not to go away mad. Jake adds, "But do go away." Isn't that a Motley Crue song?
Carter's taxi is stuck in traffic, and the driver keeps honking the horn. Carter opens the car door and gets out in the rain with his bag. He starts running through the traffic until he gets to the sidewalk, then springs down the street. Luckily, he was right by Kem's apartment building. He rings some buzzers until someone lets him in, and then sprints up a few flights of stairs. He pounds on Kem's door and rings the doorbell, but there's no answer. Until Kem opens the door. Carter says, "I'm moving back to Africa and we can start over, and do everything we didn't do before. You can take as long as you need.
