Meredith offers to run the ER for a night, and we get to spend an hour with her in almost real time to see just how action-packed an hour in the ER can be. Most of the doctors and patients alike are all trying to get done and leave so that they can make it to a big UW basketball game, this includes Meredith's first patient who comes in with mild chest pain and wants to leave so he can take his son to the game with tickets he gave the kid for his birthday. If you think this sounds unfortunately ominous, it is -- his x-rays keep coming back looking not quite right so she sends him for a CT, where he winds up coding while in the machine because he has a ruptured aorta. It's something that's almost never caught and even when it is, it's still fatal more times than not. Teddy happened to still be at the hospital even though she was trying to leave on a date, so she and Mere rush the guy to surgery, but he winds up dying anyway. Meredith then hears his cell phone ringing and answers so that she can break the news to his family. Teddy has a much lighter evening of it and winds up going on her date, though not without Henry's trying to dissuade her first. He had shown up unexpectedly for a test of some sort and wound up killing time chatting with said date, and as Teddy is about to leave he warns her that the guy seems kind of like a poncey loser. Teddy ultimately decides to go anyway but seems (at least from the glance she shoots Henry as she gets on the elevator) to at least realize that things might not be so cut-and-dried with her insurance-only husband.
Another patient who wants to get to the game is a really drunk dude who walks in to the ER with a knife sticking out of his head. He and his equally drunk buddy had gotten into a bar brawl which resulted in the knifing, but are so drunk that they just find the situation hilarious and hope that it doesn't make them late to the game. Against all odds, the knife didn't hit anything important, and while the doctors are gathered trying to figure out the best way to get it out of his head, the dudes decide to take matters into their own hands, literally, and the friend winds up just pulling the knife out. The guy is fine (and both are still very drunk), and he decides to leave against medical advice so that they can still make tipoff.
Alex and Lucy are continuing their strange little mating dance from last week where they insult but gaze at each other. Lucy's tune changes, though, when she finds out that Alex has floor seats to the game and she agrees to help Alex clean up a bunch of his charts so he can get there on time. She then manages to get him to invite her and ditch Avery but she isn't even free -- she just wanted to see if she could get him to ask her. Ultimately Alex doesn't even wind up going either; one of the waiting patients in the ER is a little boy who broke his leg and needs two casts put on, and he and his parents have been waiting all day for an open OR for the procedure. Alex gives both tickets to Avery so that he can do the casts, so Avery invites Lexie to go to the game with him and she happily accepts.
In the middle of all of the medicine, Cristina tells Meredith that Callie asked her to be the baby's godmother. Meredith gets really weird about it and then finally admits to Cristina that she feels like if Cristina accepts, it means she thinks Mere is never going to have a baby of her own to godparent. Cristina of course doesn't mean that at all, and deep down Meredith knows it, but she feels the way she feels and so Cristina decides to pass on Callie's offer. Mere's also a little bit extra emotional because she and Derek have stepped up the fertility drugs to the point where Derek is having to give her injections, one of which he gives her in the middle of her hour. When they try to slip into the on-call room to do it, Derek walks in on Bailey and Eli getting busy though he doesn't admit to Mere who and what he saw. Bailey had repeatedly turned down Eli's requests for some in-hospital nookie but finally gave in -- Derek's busting them appears to cause her to strengthen her resolve never to try sex in the workplace again.

As evening descends on Seattle, Meredith's voiceover asks how much we can accomplish in an hour and throws out suggestions including running errands and sitting in traffic. If you're in L.A., you can do that last one for far longer than an hour if you want to. To illustrate an hour we see sped-up footage of doctors and nurses working away in the ER, and then we see the clock tick over to 6:00 PM. Mere explains that in medicine they have a term called "golden hour" which refers to the magic window that determines whether a patient lives or dies.
As the 6:00 hour begins, Mere writes some things up on the patient board. Since this is done in real time we're treated to a bit of filler at a few different points during the episode. Cristina is kicking back at the desk eating potato chips and declares that she's not going to help out since she just finished four back-to-back surgeries and this is the first time she's sat down or eaten all day. She also reminds Meredith that she got herself into this situation since Mere volunteered to help run the ER. Mere gives some people some directions and then explains to us/Cristina that she thinks this will make her look like a good candidate for Chief Resident. She's totally giggly and smiley and Cristina notes this suspiciously; Mere explains that she's on fertility drugs and therefore full of extra hormones that are currently making her attitude and her boobs extra perky. Cristina then delicately tells Mere that Callie mentioned "a thing" but she's interrupted by Lexie, who has a patient with a migraine ready for discharge. Mere makes sure she ran all of the appropriate tests and Lexie assures her that she did, and comments that the most interesting thing about his case is his horrifying girlfriend from hell. They all turn and stare at a redhead busy talking on the phone to the poor guy. Mere signs off on the paperwork and Cristina tries to pick up where she was interrupted but they are then interrupted by Arizona and Owen who have patients. Mere, though, seems to be doing a great job wrangling everybody. Once those interruptions are through, Cristina finally admits that Callie asked her to be the baby's godmother. Meredith's not happy about it and asks if she said no, and says that Cristina has to say no like it's the most logical thing in the world. Cristina's taken aback but Mere heads off to meet a new patient so they can't discuss it yet.
Her patient is Oliver, a good-looking businessman who is on his phone, explaining to his wife that his coworkers overreacted when they heard he was having chest pain, and he's that it's just indigestion. Nurse Tyler is around as Mere's right hand man this hour and he removes electrodes from Oliver's chest as Oliver tells his wife to have their son put on his basketball jersey, and he'll be home soon. He's hot, nice, and has a kid waiting on him? I smell doom for poor Oliver. He actually apologizes to Mere for talking on his phone, and that gesture just insures that something horrible is going to happen. He's supposed to be taking his 9-year-old, basketball-crazy son to the University of Washington basketball game that night with tickets he got the kid for his birthday. He had some chest pain but insists he's feeling better, though Mere realizes this might be his way of trying to get out earlier. Meredith assures him that she will try and get him checked out and through his tests as quickly as possible, and while he's visibly disappointed he grudgingly understands that she can't just take chest pain lightly.
When Meredith heads back out Cristina wants to talk about the godmother thing but Mere just has her look at Oliver's EKG results, which seem normal. Alex walks up to the desk and though he's supposed to have the night off, Meredith tries to put him to work. He flat-out refuses and explains that he's just there because if he doesn't take care of all of his unsigned charts he's going to have his privileges suspended. The week prior he told a patient that he didn't have cancer and the guy was so happy he gave him two tickets for floor seats to the very same basketball game Oliver is planning to attend. Meredith and Cristina are both fairly appalled to hear that he took a gift from a patient but he ignores them, and Cristina gets paged for surgery. A man comes up to ask about an update on his 4-year-old named Nathan, who needs a cast on his leg. Mere realizes that their doctor hasn't been to talk to them so she has to explain how, because the boy has a broken femur, he'll need a cast on the broken leg attached to a cast on his good leg to keep everything stable so it can heal. As if this wasn't enough, Nathan is so young that he has to be put under to do so, and therefore they need to do the procedure in an OR but all are currently full. Meredith assures him that as soon as one is available she'll let him know and while he's visibly overwhelmed, he thanks her. She then turns to Alex and begs him to stay and help, but Alex has eyes only for his floor seats.
Meanwhile, Eli seems to have offered Bailey some kind of indecent proposal, because she's as giggly as a schoolgirl as she assures him that while lots of people in the hospital take part in what he's suggesting, she isn't one of them. He and his dimples just smile seductively at her and tell her to consider whatever it is, adding that he's sure it would be exciting and "toe-curling." It's obvious that it's some form of the horizontal mambo but the question is what circumstances have caused Bailey to very weakly attempt to decline. Meredith walks by and she tries to compose herself but once she passes, Bailey whacks him with a paper in that flirt-fighting way we girls have.
Lexie is giving her patient Mitch a prescription for migraine meds before she releases him. (Fun factoid: Mitch is played by Jeffrey Doornbos who used to be a Blue Man with Blue Man Group in New York. Somewhere, Tobias Fünke is filled with jealous awe.) His girlfriend, Gia, proves she's every bit the bitch Lexie had mentioned earlier by giving Mitch a guilt trip that they're missing their anniversary dinner just because he had a headache. She reminds him that it was really hard to get reservations, but Mitch seems more annoyed than guilty and tells her he's sorry but his head really hurts. Lexie, meanwhile, is trying not to stare at the woman though she's obviously judging her harshly.
Meredith pulls back a curtain to greet her patient and is shocked to see that it's none other than Adele Webber holding her wrist. She tries to tell Meredith it's not a big deal but that when she got home from work she slipped off of a stepladder and landed on her wrist. Despite what she says she can't move without wincing, and Meredith doesn't agree when Adele assures her that she can wait while Mere sees more pressing patients. Mere replies that as the Chief's wife Adele herself is her most pressing patient but Adele looks over Meredith's shoulder and asks, "Are you sure about that?" Meredith turns to see two incredibly drunk guys walk in, arguing with each other. One of them then turns to face the room and asks if anyone can do something about "this." "This" being a giant knife buried up to the handle in his skull, just over his ear.
So it turns out that Adele was right that a dude with a knife in his skull takes precedence over even the Chief's wife. Derek's in brain surgery but Mere goes in to tell him what's up and to ask for him to consult when he's done. Avery's pretty stoked at the idea of a human knife block but tries to act concerned when Derek and Meredith glare at him for his callous cry of joy, and Derek sends him off to get a CT. He then reminds Meredith that they have a date at 6:30. He might want to close up the dude's head in front of him, then, since it's 6:13 according to the OR clock.
As they walk, Mere explains to Avery that the knife block, Stewart, was in a brawl at a tailgate party, which I imagine is for the same game everyone else is all excited about. As Avery takes off Meredith calls after Bailey, who is about to enter the on-call room. Bailey freaks out and yells that she's about to take a nap, and she goes maniacally into so much detail about how she's merely going to sleep that it's obvious that Eli's suggestion was that they hook up there. I know he really wants to do it but I'm not sure Eli understands truly how big moment this is for Bailey, who considers it a point of pride that she's never stooped to the level of getting busy in the hospital. Mere's completely baffled by Bailey's babbling, especially when she starts shaking her head strangely -- a sign for Eli, who had just rounded the corner for their rendezvous. All Meredith wanted to know was if Bailey had seen Teddy -- which she hadn't -- but Mere spots Teddy herself and takes off. Eli then finally walks up and makes a great show of yawning before holding the door open for Bailey and following her inside.
Mere wants Teddy to do a quick consult and she only grudgingly agrees, as she was supposed to be off work at 6:00. She looks downstairs and sees a good-looking guy in a sports coat and curses that he's early before asking Meredith for a moment and jogging downstairs.
Sport coat's name is William and he's there to take Teddy out on a date. She's embarrassed to still be in scrubs but William actually loves it and assures her that he has no problem waiting while she wraps some things up. So of course, what should happen ? Henry shows up at the hospital for an unexpected test of some sort. Teddy tries to stay cool and Henry quickly realizes what's going on as he's introduced to William. William asks if he's a patient of Teddy's and Henry actually plays along, answering that he's "something like that." Teddy seems grateful to be able to escape to her consult, though this leaves the two guys to hang out in the waiting area as Henry has to wait an hour for his test. It's a testament to Henry's awesomeness that he realizes what's up and plays along seamlessly, but Teddy is still freaked out.
Teddy takes a look at Oliver's films and Meredith points out that something on the x-ray seems not quite right. Teddy has Meredith order some labs and says that she'll see Oliver but she wants to make it fast so she can go on her date with hot William.
Alex is at the desk with his giant stack of charts, and he calls after April to ask if she'll forge his name on some of them. April is disgusted by his question and she lectures him on the immorality of forgery. As she storms away Lucy walks in and, having overheard, claims to not be surprised that Alex would stoop to forgery. He doesn't argue with her since his only concern is getting to the game, and on hearing that her ears perk right up. She explains that this is the Pac-10 title game and that if UW wins they'll get an automatic bid to March Madness. Hmm, I wonder if the show paid for that name drop, since that term is trademarked and that's why you see all sorts of advertising for "March Mayhem" parties and the like. Also, while I appreciate that they are showing that she knows and loves basketball everyone knows that conference championship games aren't played in February! Regardless, Lucy grabs the charts since she has time to kill and thinks it's important that he get to the game. Alex is shocked and points out that he thought she thought he was a loser, but she admits she thinks he's less of a loser now that she knows he likes college basketball. She then sneaks a look at him and grins to herself.
Cristina is back since her surgery was postponed, but she's stoked that she now gets to get to see the sideshow that is Stewart and his knife. As she and Mere ride the elevator Mere declares that being a godparent to Callie's baby is a bad idea and she thinks Cristina doesn't want to do it. "I don't?" Cristina asks, so Mere explains that three people have to die before she can step up and fulfill her duties for this baby while if it's Mere's baby, just she and Derek need to bite it for Cristina to take over. Even with her twisted sensibilities, Cristina's totally disturbed by this line of thinking even though Mere just chattily explains, "One wrong turn down a dark and twisty road and BOOM, you're in." No matter how well-adjusted Mere has become, there's still going to be that dark and twisty place inside her, it seems. Cristina tells Mere that either way, she only ever intends to be a godparent in name only -- this is why boarding schools exist. They then make their way to the window to watch the farce currently taking place in Stewart's room.
Stewart and Dave are enjoying beers but when Lexie and Avery walk in she disgustedly grabs them away from both of them. Cristina goes to join them as Oliver is rolled by, and Meredith goes to speak to him. He pleads with her a little to try and get out of the tests but she tells him they have to do it, and promises to make it as fast as possible. As he's wheeled on, she hears Richard yell her name and he sounds the very opposite of pleased.
He's bent out of shape on account of the fact that he learned Adele was in the hospital from a scrub nurse asking how she was doing, rather than from Meredith. Adele is behind him, rolling her eyes at his theatrics, and she tries to quiet him and tells him that Meredith hadn't even had a chance to examine her yet. Richard asks warily if this is true and Adele just continues on by explaining that when he gets worried, he gets gruff. She then once again explains that she hurt her wrist by falling off of a stepstool, but this time adds that she had been reaching for the holiday decorations. Neither Richard or Mere mentions that this is a bit strange, given that it's the middle of February and unless she likes to decorate the house with top hats, glorious beards and festive curled wigs for Presidents' Day, there isn't really a decorating holiday coming up anytime soon. Meredith's face is extremely serious as she assures Richard that she ordered and x-ray and an ortho consult, and that she'll call and make sure it's coming soon. How would she have ordered those things already if she hadn't yet examined Adele? I'm not sure, but Richard doesn't mention it and they are interrupted by a chorus of screams which Mere leaves to investigate.
The screams, of course, came from Stewart's room and were from all of the gathered doctors -- Stewart had been showing off how when he moved his eyebrows, he could make the knife in his head move too. He does it again to show Mere and the docs all holler at him to stop moving. Dave then pipes up to brag that the fight was awesome since Stewart kicked a guy's ass. I think that guy had the last laugh burying a knife in Stewart's melon, but then again maybe not -- Cristina asks him math questions and while he doesn't get anything correct except for the simplest one, he points out that he missed those because he's drunk, not because he's got brain issues. Amazed, Cristina agrees that his mental status is totally intact, as if there was no damage at all. Dave wants to just pull the knife out so that they can get to the game, and then he pulls another beer out of his pants to keep the party going. Lexie shrieks and grabs it away but he just finds another one. He's like a clown car with the way he continues making bottles appear as if from nowhere.
Meredith goes over to Tyler to review the status of all of their patients and then asks to see the charts of everyone who was recently discharged. The first one she looks at belongs to Mitch, Lexie's migraine patient with the awful girlfriend, and Meredith freaks out over what she sees and runs out to the parking lot to see if she can still catch him.
He's not moving terribly fast and so she gets to him before he's gotten into his car. Immediately she asks him to smile, and after a moment of struggling he's only able to lift one side of his mouth. She then asks him to lift his arms, but he can only lift one. Gia's more ticked off than worried at all this commotion but when Tyler runs out, Meredith tells him they have a code somethingbad and orders him to get a stretcher. She asks Mitch to repeat a simple sentence and he can't really form the words, which causes Gia to finally start worrying about something other than her ruined evening. Mere orders the woman to help her lower Mitch to the ground and as she shoots orders to Gia she informs her that Mitch is currently having a stroke.
Once they have him inside, Mere tells some random doc that his symptoms started 20 minutes ago so they are well within the "golden hour." Gia, meanwhile, is on the phone and is more upset that Mitch could have been driving her when this happened than she is about him having a stroke in the first place. Amazingly, though, this isn't her most self-centered thought of the hour. She outdoes herself when Meredith comes over and explains that since they caught this early they think he'll be eligible for a particular stroke medicine that should fix him up. Gia asks her how he's doing on a scale of 1-10, but defines 10 as needing to cancel their upcoming Aspen trip. Mere replies, very slowly as if Gia's mental capacity is, well, a little bit smaller than the rest of ours, that she should cancel the trip. Gia then gets back on the phone and begins to tell her friend what's up, all put out. While I'm sure a girl this idiotic would have idiotic friends, I'm still shocked that whoever is on the other end of the phone hasn't told her to get a grip yet.
Mark's taking his turn examining Stewart; his theory is that the knife itself is preventing any bleeding and they are going to have to crack open Stewie's head to get it out, but Jackson comments that they could try just pulling it right out. Dave's ears perk up at this but Mark berates him for what he thinks is a stupid idea and guesses that Avery just wants to get off work to go to the game. Lexie speaks up and defends him, though, pointing out all of the reasons why taking out the knife might just work and would be less invasive than doing a full-on craniotomy. Mark knows that's not why Avery made the suggestion but they move forward pretending that it was. Avery is honest enough to add that he does also want to make it to the game on time, though. Mere then pokes her head in and in her best no-nonsense voice tells Lexie they need to talk.
Lexie's so relieved to get away from Mark that she doesn't notice that Meredith has her Professional Serious Face on, and she doesn't find anything remiss on the chart when Mere hands it to her and asks if she sees anything wrong. Mere finally points out Mitch's signature when he came in versus the one when he was discharged, which amounts to a squiggly line that droops down to the bottom of the page like it's trying to escape the paper and is making a drunken run for it. Lexie finally realizes what's wrong and Mere tells her about the stroke, then points out that she knows Lexie knows the symptoms, so was she distracted by all the Mark stuff? Lexie maintains that he had classic migraine symptoms and that he couldn't have been having the stroke when she was treating him, but that sad signature line tells another story. She's saved further lecturing, though, because Callie walks by and yells at Meredith for not telling her that her consult was for Adele, since she would have come down faster if she knew. Everything seems to be weighing heavily on Mere's shoulders, and she shuffles off after Callie to go see Adele.
Adele tells her story to Callie, who informs her that she actually has a hairline fracture, but that she'll splint her up and send her on her way. Richard then gets paged and takes off as well, leaving Mere and Adele alone, and Meredith thanks Adele for covering for her earlier. Adele has no idea what Mere is talking about, and Mere realizes she doesn't even remember that happening. Very casually, Mere asks her if she remembers hitting her head when she fell but Adele does not. When Tyler comes over with Oliver's scans, she quietly tells him to order a head CT for Adele. She then looks at the scans, swears, and runs off.
Teddy has changed for her date and is looking hot and Amazonian in tall black heels as she walks up to William and Henry, who have been visiting all this time. William is impressed to learn that Henry played professional baseball for a short while, and Teddy grins and grits her teeth as she admits she didn't know that about him. Henry starts to send them off on their date but Meredith runs up to show Teddy Oliver's scans, which have something on there bad enough for Teddy to again have to postpone dinner. She offers to reschedule but William assures her that she's saving lives, and the least he can do is take her to dinner when she's done. William reminds me of a hot Ed Helms so I kept expecting him to suddenly be wacky, but he's just extremely earnest and in awe of Teddy. He sits back down with Henry while Teddy tells Mere to get a CT. They hear giggling from somewhere as they walk down the hall; I thought at first that these were delighted squeals coming from the on-call room but they are actually coming from Lucy, who seems to be having a ball helping Alex with his charts.
Mere goes back in to Oliver's room and dashes his hopes that he might still make it to the game on time. As she's explaining that he's going to need a CT his phone rings and of course his ringer is the sound of an organ playing at a sporting event while the crowd cheers, so that Mere can really be reminded that this will keep him from taking his son to the promised event on time. Oliver is sad but covers as he answers the phone and assures his son that Daddy will be fine.
Back at the circus that is Stewart's room, Owen, Avery, Lexie and Mere are just outside looking at his x-rays and marveling at how the knife seemed to not hit any brain matter. They agree to wait for Derek to make a decision about how to remove it, but Dave and Stewart have other plans. The doctors are too busy staring at the films to notice that Dave has his hands on the knife, and after a countdown, he pulls it out. Awesomely, Stewart just shakes it off, and Dave goes to the window and taps it with the knife, asking if they can go to the game now. Behind him, Stewart gives an enthusiastic thumbs up to the dumbfounded docs.
As the second half of the episode begins we're given another shot of the clock showing that it's just after 6:30. Meredith and the others are checking out Stewart, who seems to be none the worse for wear after having a six-inch blade pulled out of his skull. Dave is playing with the knife and Lexie grabs it away from him, continuing her role as angry mom/fun killer. Dave glares at her appropriately. Derek comes in to check him out, but Meredith hears Nathan cry and runs over to see what's going on.
She runs to his bedside and promises it will be okay and she'll get him more pain meds, and his mom assures him it will only be a little while longer. She then asks Mere if this is actually true, and Mere passes the buck to Tyler by demanding to know the status of the OR. She has him call again and make sure the team is ready to go. Derek then comes out of the room and after they briefly talk about how Stewart just might be the luckiest man alive, Derek reminds her that she's late for their date. Mere insists she can't leave but Derek realizes she's just waiting on a whole bunch of things right now and she finally agrees she has one minute she can give him. He assures her that's all he needs. Derek... is that something you really want to admit?
Meredith giggles as he pulls her down the hall to the on-call room. Derek is the first one in and opens the door to the two of them in quite the compromising position, and Bailey lets out a scream. I have to imagine as soon as he shut the door she also then hit Eli with a pillow and started telling him, "I told you so," until he managed to turn the subject back to his hot body currently intertwined with hers. Derek tells Mere the room is occupied and then adds, "Never ask me what I might have seen or might not have seen in there." He's actually really cute as he says it and I find myself having strange and uncomfortable feelings of finding him charming for the first time in several seasons.
Some wacky music kicks in just to insure that we realize hijinks are afoot. They get into the elevator and Derek tells her they're doing "it" right here, pulls the emergency stop, and instructs her to drop her pants. Seriously, they had me. I thought he wanted to have another go at making a baby and he really figured he only needed one minute. She protests mightily but finally agrees to it, and he reaches into his pants and pulls out... a syringe full of fertility medication? The elevator has always been a special place for these two and now it's the place where Derek literally jabs Meredith in hopes of making a baby. He then tells her that he can't wait to meet their future fictitious baby, sets the elevator to go again, and then hops off at the floor.
As Meredith leans back to let the hormones permeate her hostile womb, Alex and Lucy get on the elevator and she points out that she just helped insure he will make it to the game on time, so isn't he going to invite her along? Alex lets his guard down and decides to drop Avery like a hot potato in favor of taking Lucy. However, Lucy seems to be acting bitchy to try and cover up her own budding feelings for Alex and informs him she's not available, but she just wanted to see if she could get him to ask. She then gets saunters off the elevator and Meredith watches Alex turn pink, then try to cover up his embarrassment with a look of disgust.
Teddy finally comes back downstairs for her date and as poor Oliver is dead, it seems that this time she'll probably really get to go on it. But while William wraps up a phone call, Henry pulls Teddy aside to give her the rundown on what he's learned about William in the last hour: he wears driving gloves, he lived above his mother's house until just recently, and he used the word "shan't" without any trace of humor or irony. Teddy looks a little shellshocked to get all of this news in one swoop, as any girl would be to hear that her date wears driving gloves. Henry tells her that as her husband, he thinks "we can do better." The first time I watched this scene, I took that to mean that he's on her side in this. But the second time I watched, I wondered if he might also mean that he and Teddy would make a better couple than Teddy and Mr. Shan't. William walks up just then to tell her that he got their reservation changed -- I like to think they got the reservation that Gia and Mitch had to cancel -- and they turn to leave. She looks a little overwhelmed at all of the info, and as she walks into the elevator she turns back to look at Henry. Mere tells us that an hour can change your mind. Maybe this means she's picking up on what we all know, which is that she and Henry could be the cutest couple on the block if she'd give it a chance.
Bailey and Eli sneak out of the on-call room, and she's totally giddy after their "nap."
